An SNP, or Special Needs Plan, is a Medicare Advantage plan that offers coverage to people with End-Stage Renal Disease (ESRD), chronic diseases, and other debilitating conditions. Additionally, SNPs also offer benefits to people under 65 who qualify for the plan’s terms. Normally, only people above the age of 65 can receive Medicare benefits. Plans can be customized to meet the needs of each individual.
SNPs are also available to seniors who live in long-term care facilities, such as nursing homes.

For people who have disabilities, End-Stage Renal Disease, or low-incomes. A special needs plan is a unique Medicare Advantage option as it does not require that beneficiaries are age 65 or older. Instead, anyone who has a qualifying debilitating disease or disability and falls within certain income tiers can qualify for an SNP.
